If you keep a mission database, (see examples and explanations see Database of Databases) you may wish add information about your database to the Database of Mission Databases using the form below. The Database is maintained by Steve Hayes (address below) on behalf of IAMS. Information from the database is available from him at methodius@bigfoot.com

bulletSimply fill in the blank spaces in the form and submit the message.
bulletPlease don't alter the field names in any way - the fields are demarcated by square brackets [ and ].
bulletType your information BETWEEN the square brackets.
bulletIf you don't have information to put in a field, leave that field blank - DON'T write "n/a", and DON'T delete the field name.
bulletTry to keep entries to one line, except for the description[ field, where you may type 6-10 lines.
bulletSee the notes for the other fields after the form.

NOTES ON FIELDS

Researcher[ - This should be the name of the person responsible for keeping or maintaining the database.

Institution[ - if the database is an official project of an institution, such as a university or seminary, put its name here, otherwise leave the field blank.

Postaddresss[ - the postal address of the person named in the Researcher[ field. If you need more than one line, press the <Enter> key before you reach the end of the line, and preferably add a right square bracket ] at the end of the information.

Web[ - if you have a Web page, put its address (URL) here. DON'T forget the "http://" - it is an ESSENTIAL part of the address! If you don't have a Web page, LEAVE IT BLANK!

Subject[ - this should be the name of the database, a brief description of the contents, like "Bibliography of Pentecostal missions in Brazil" or "Biographies of Asian church leaders". Save more detailed descriptions for the Description[ field.

Geogarea[ - the main geographical area covered by the database - country, region, or continent. If it is unrestricted, put "worldwide".

Churchmission[ - if the database concentrates on a particular church or mission, or group of churches or missions, put it here, eg "German Lutherans", "Society of the Divine Word", "Church Missionary Society" etc.

Software[ - The software you use to store the information. This may be a computer program - e.g. dBASE IV, or some other medium such as "3x5 cards" or "microfiche".

No_entries[ - the approximate number of entries in the database

Keywords[ - you don't have to put anything here. The software I use, askSam, can search all words of any and every field, so you should try to incorporate important key words in the Description[ field. If there are any that you haven't managed to fit in, put them in the keywords field - things like acronyms.

Description[ - as you can see from the reports above, the Description[ field contains the main information about the database - the kind of information it contains, how you can get access to it, and whether and how you can contribute new information.
